Why the test comes around every year
Backflow is water moving the wrong way. When pressure drops in the supply line, or pressure builds downstream, water from a hose, a boiler, an irrigation system or a sink can be pulled or pushed back toward the clean supply. A backflow preventer is the valve assembly that stops it.
Three plain reasons the test is not optional. City codes require backflow testing to be completed yearly. Backflow preventers can get damaged unexpectedly. And water that's been exposed to backflow is unsafe to drink.
A preventer that looked fine last spring can have a fouled check valve this spring, and nothing about the faucet in the kitchen tells you. The yearly test is how anyone finds out.

What a test involves
A certified tester connects a test kit to the test cocks on the assembly and checks that the check valves hold and, on assemblies that have one, that the relief valve opens when it should. The assembly either passes or it needs repair, and a repair is a plumbing job like any other.
The same visit works for a single-family home with an irrigation system, an apartment complex with several assemblies, an office, or a larger commercial building. Properties differ in how many assemblies they have, and that changes how long the visit takes, not what the test is.
